WORTLEY MEN'S CLUB

To become a member, please complete a Membership Application form which can be found at the bar in the club or downloaded below. Please complete all the details requested on the form in capitals, so we can get the details right, and hand it in at the bar.

The form will then be presented at a committee meeting and the application will be approved or rejected. If approved you will be contaceted and a welcome pack will be left behind the bar. You will have approximately One month to pick up the pack and pay the appropriate fee. This will then allow you to use all the facilities of the club and be entered into our Weekly Free Prize draws.
